We are pleased to confirm that the flu vaccination will be offered at St Peter's on Wednesday 20 November 2024 to help protect your child. This is free from the NHS.
Please complete the online consent form here by Friday 15 November, using the unique Session Code: 5hxazc
If you do not wish for your child to receive the vaccination, please complete a NO CONSENT so that we do not contact you regarding the Flu vaccination this season.
If you miss the deadline, you can still complete a form but will be offered an appointment for a catch-up clinic at a community venue.
